The Role
We are looking for a tech-savvy individual with a passion for health and older adult care and a clinical and operational leader mindset to join our growing team. Isaac Health offers medical consultations, therapy sessions, and cognitive assessments for the diagnosis and treatment of Alzheimer’s and Dementia Related Disorders (ADRD). The position will join our team to help us provide outstanding patient care through our innovative telemedicine platform. In this role, you will be responsible for overseeing our Care Navigation team, managing the day-to-day operations of our clinic, and ensuring that we are providing the highest level of care to our patients and their loved ones. This position will interact with and help manage multidisciplinary clinical providers and other members of patients’ care team to coordinate their care.
The ideal candidate will have a strong background in clinical management, project management, and care coordination. You will also be a highly organized and detail-oriented individual with excellent communication and interpersonal skills.

Experience with people living with dementia or Alzheimer’s preferred.
- Tech Savvy: You know how to troubleshoot your own system and can help others with issues if they arise.
- Able to provide own personal computer/laptop that meets security and telehealth standards
- Independent: You thrive in a remote environment.

- Keen attention to detail to ensure accuracy in administrative tasks, compliance with healthcare regulations and Isaac workflow protocols, and the delivery of high-quality patient care
- Efficiently managing time to balance various responsibilities, including administrative duties, staff supervision, and patient care coordination
Preferred Qualifications
- Bachelor's or Master's degree in Healthcare Administration, Business Administration, Social Work, Counseling, or a related field.
- 5+ years in healthcare management or clinical operations.
- Clinical experience in social work and counseling, especially with older adults.
- Proven ability to train and guide Care Navigators.
- Strong understanding of healthcare regulations and best practices.
- Excellent organizational and time-management skills.
- Ability to work effectively in a remote environment.
- Proficiency in telehealth technology.

Job Objectives
- Oversee daily clinic operations, including scheduling, patient flow, and administrative tasks
- Manage provider schedules and escalate concerns regarding supply and demand
- Oversee and manage a team of care navigators and home care aides including, but not limited to:
- Training, onboarding and people management
- Delegation of tasks and responsibilities
- Performance management and evaluations
- Manage service level agreements (SLAs) and performance metrics including but not limited to:
- Managing task/work volume
- Ensuring timely coordination of care for our patients
- Appointment volumes
- Ensure the quality and performance of the team is of an exceptional standard
- Assist with provider onboarding, ensuring they are set up for success
- Process provider and hourly contractor payroll
